Sutter Health and Alameda Health System Partner to Improve Care for Frequent ER Patients

For the first time, six East Bay hospitals, including Sutter Health’s East Bay region, are partnering to improve healthcare for frequent Emergency Room patients. The participating hospitals include Alta Bates Summit in Oakland and Berkeley, Sutter Delta, Eden Medical Center, Highland Hospital in Oakland and San Leandro Hospital in San Leandro.
With the implementation of the new PreManage ED system, this joint effort between Sutter Health and Alameda Health System provides a secure, portable health record and care plan for each patient. It is the first data sharing initiative among Northern California hospitals with a common goal of coordinating care for frequent ER patients.
Funding for the new system used philanthropic investment from Better Health East Bay and a $75,000 grant from the California HealthCare Foundation. Additional partners include Collective Medical Technologies, the developers of PreManage ED; the medical teams at Sutter Health and Alameda Health System and the Sutter Research, Development and Dissemination team.
